Abstract:

Cyclotron operated within a hospital to deliver radiation to produce short life radioactive isotopes for diagnostic purpose is used in many hospitals. But cyclotron used for proton therapy is just at beginning. Proton beam therapy is a technologically advanced approach since it achieves precise radiation dose distribution. As the depth of penetration is a function of its energy, the point of energy release of the proton can be very precisely determined. The path of the proton beam is linear and causes very little secondary lateral scatter. Healthy tissues upstream from the tumor receive only a small dose and those located around the tumor and behind receive virtually none. Thus proton beam therapy is the most effective method. On the other hand, the cyclotron must be reliable, and the beam must be stable to ensure patient´s safety since the proton beam is used directly. Efficiency of beam delivery to the treatment rooms, raster scanning techniques to ensure the correct control of beam energy, intensity, and position stability, rapid and precise flexibility in changing beam parameter are also essential for satisfying clinical needs. The design efforts for developing a hsopital-based medical therapy cyclotron including the clinical beam delivery systems and integrated control system are really complex, and are less well developed than patients real need. A conception scheme for the purpose is proposed.

Clinical response with functional image during radiotherapy of esophageal squamous cell carcinoma

Abstract:

18F-FLT uptake can be used to monitor the biological response of esophageal SCC and normal tissue to RT. Increased uptake of 18F-FLT following treatment interruptions may reflect accelerated repopulation. 18F-FLT PET may have advantages compared to 18F-FDG PET in differentiating inflammation from tumor. 18F-FLT PET may help oncologists predict treatment response as early as one week after the beginning of treatment and estimate clonogenic repopulation during RT. 18F-FETNIM PET can depict hypoxia in esophageal SCC. Prior to CCRT, tumor hypoxia demonstrates spatial variability in different days, though overall 18F-FETNIM uptake remains similar. Baseline SUVmax may be predictive of treatment response.

Abstract: We implemented a new protocol — multiphase dynamic helical scan to acquire CT angiography (CTA) and whole brain CT perfusion (CTP) images simultaneously with single scan on 16 multidetector CT (MDCT). A total of 90 patients who were randomly assigned into 3 groups were included in our study. Each group underwent CT scan by using the new protocol, traditional CTA and CTP protocol, respectively. The image quality of CTA, the CTP parameter values and the X-ray doses were measured and compared between the new protocol and the traditional protocols. There was no statistically significant difference in the CTA image quality between the above methods ( =0.55). For CTP parameters, the new protocol tended to overestimate the blood volume (BV) and blood flow (BF) value, and to underestimate the mean transit time (MTT) value compared with the traditional method. However, there was no statistically significant difference in BV, BF, and MTT value between the two methods except permeability surface (PS) ( >0.05). The volume CT dose index (CTDIvol) and dose length product (DLP) of our protocol were lower than the traditional one. The new protocol can obtain valuable diagnostic information in a shorter time without significant compromise in image quality. In addition, it reduces the radiation dose as well as contrast medium usage on the patient.
